Late Registration:
Forms Required for Registering:
- Two (2) Proofs of Address
- Copy of Social Security Card
- Copy of Birth Certificate
- Copy of Immunization Record
- Request for Records form - Signature
- Copy of Current report card or transcript
- Signed Affidavit (must be completed at BCS with a BCS witness)
- Completed Exit Form for the school your child is leaving
- Signed copy that you and your child read the handbook (Download the handbook at the links below.)
Students Exiting
