Forms Required for Registering:
- Two (2) Proofs of Address
- Copy of Social Security Card
- Copy of Birth Certificate
- Copy of Immunization Record
- Request for Records form - Signature
- Copy of Current report card or transcript
- Signed Affidavit (must be completed at BCS with a BCS witness)
- Completed Exit Form for the school your child is leaving
- Signed copy that you and your child read the handbook (Download the handbook at the links below.)

Students who EXIT Beekman Charter School must fill out an exit form in the office prior to entering a new school.
